WE BELIEVE YOUR HEALTH IS A PRIVATE MATTER
Your personal health information collected by both Winnipeg Regional Health Authority (WRHA) and Youville Centre staff is maintained in one file.
We collect, record, store, use or disclose any facts about you and your health in keeping with Manitoba 's Personal Health Information Act.
Personal Health Information :
These are any facts we collect to help provide health care or payments for health care. They could include:
- Your name, address and Personal Health Identification Number (PHIN);
- Facts about your health, health care history and the care you have been given;
- Facts about payment for your health care.
We will use this information in keeping with the Act, sharing it only with those who need to know. For instance, we might use it to:
- Provide and check on your health care;
- Teach and train health care students;
- Do research and planning for health care we provide.
These are your rights under the law:
- You may see or have access to your personal health information.
- You may ask for and receive a copy of your health records.
- You may ask us to correct your records.
- Your information is private. Unless sharing it with others is authorized by law, we cannot and will not give out any of your information without your consent.
- You may make a complaint to Manitoba 's Ombudsman's Office (telephone 982-9130) about access to your personal health information, or about how it is collected, stored, used or disclosed to others.

Information Integrity - We ensure the information we have is properly maintained and we take careful measures to ensure it is not lost or inadvertently destroyed. Youville Centre never sells or trades personal information.
Security - Your information is stored in a controlled, secure environment, protected from unauthorized access in a number of ways, including passwords.
Limited Access - Staff and volunteers are authorized to access personal information only based on their need to deal with the information for the reasons for which it was obtained and are under strict confidentiality obligations when they do so.
