Our Community Health Nurses and allied health teams are dedicated to meeting you where you are at and can be a vital link in the health and wellbeing of individuals and communities!
The program focuses on improving the lives of diverse communities through education, prevention, and treatment. We specialize in providing sexual and reproductive health and episodic care to individuals who face barriers to accessing safe, timely and non-judgmental health and social services!
Our Community Wellness Clinic
- Offers episodic/sporadic but wholisitc healthcare services to community members.
- Our Nursing team includes: Nurse Practitioners, Registered Nurses and Registered Nurse Authorized Prescribers.
- We offer important services to folks who may not have access to other healthcare and social services or supports.
- We can help you address your immediate concerns and get you connected with regular or specialized care if that is what you want or need!
- We also specialize in Sexual and Reproductive Health Care (STI testing/treatment, basic wound care, pregnancy supports, pap tests, birth control etc.) and provide this care in an understanding, friendly and kind way. We don't need to rush and will spend the time with you!
- We offer immunizations, injections, basic health assessments (blood pressure), health education, information, referrals and resources wherever possible.
- We offer complex care coordination, system navigation and advocacy.
