Community Health Nurses are dedicated to meeting you where you are at and can be a vital link in the health and wellbeing of individuals and communities!

 

These registered nurses focus on improving the lives of diverse communities through education, prevention, and treatment.

 

Our Ask A Nurse Program: 

  • Offers episodic, sporadic but wholisitc healthcare services to community members.
  • Our Nursing team includes: Nurse Practitioners, Registered Nurses and Registered Nurse Authorized Prescribers.
  • We offer important services to folks who may not have access to other healthcare services or supports.
  • We can help you address your immediate concerns and get you connected with regular care if that is what you want!
  • We also specialize in Sexual and Reproductive Health Care (STI testing/treatment, basic wound care, pregnancy supports, pap tests, birth control etc.) and provide this care in an understanding, friendly and kind way. We don't need to rush and will spend the time with you!
  • We offer immunizations, injections, basic health assessments (blood pressure), health education, information, referrals and resources wherever possible.
